Pre-admission department

If you need help or guidance in completing your digital preoperative questionnaire, before your admission, please contact the pre-admission department.
The pre-admission department is located on the ground (0) floor of block A and is available Monday to Friday between 9 a.m. and 5 p.m.

Plan your appointment


Your general practitioner (GP) or attending doctor has already discussed your admission with you. If you need help preparing for your planned admission, you can contact the pre-admission department.

During a confidential conversation, they will provide you with more specific information about your admission, ask you about your choice of room, explain any supplements and, together with you, complete the administrative formalities.

The hospital tries to consider your room choice as much as possible. However, due to the busy occupation this is not always possible.

On the day before your hospitalization, you can inform the pre-admission department in which room you will be admitted.

 

Where do you have to enroll?

The pre-admission department is located on the ground (0) floor of block A and can be contacted from Monday to Friday between 09:00 and 17:00.

What will you bring?

Bring all necessary documents with you: your identity card, proof of hospitalization insurance and/or forms from your hospitalization insurance, recent list of home medication (e.g. through your GP, White Yellow Cross, pharmacy, WZC, ...) and your home medication in original packaging. 

Home medication 

The anesthesiologist will review your file and, if necessary, give instructions on what medication you may or may not continue to take.

If there are ambiguities about your home medication, you may be called by a pharmacy assistant from our hospital a few days before your surgery. Keep a list of your home medication and your home medication itself at hand.
